Have a high index of suspicion for a pathologic rib fracture in patients with known cancer (especially prostate, breast, or myeloma) who present with pleuritic chest pain, though occasionally metastasis is the initial symptom. Suspect bone disease if the chest pain is very focal to one spot on one rib. Percuss or compress the rib at a distant spot, looking for tenderness radiating to the symptomatic area.
